Letter to Patients
Thank you for your interest in our office. Because of the amount of lecturing
and teaching which Dr. Rex
does both nationally and internationally there are some conditions that may be
different in our office than
with other practitioners you have encountered. First and foremost we believe
that each one of us is
responsible for our own health care. If you are looking for care on demand our
office is probably not what
you require. In addition to providing Osteopathic manipulative care, we see the
role of our office is to
provide you with information that will allow you to make more informed
decisions concerning your health
and enable you to become more self-reliant.
This does not mean you will never need to seek health care. Instead it means
that hopefully you will
learn how to make decisions on your own which will allow you to live more
comfortably with any
musculoskeletal problems which you may have. While there are times when you may
need to receive
Osteopathic manipulative care, there are many things that you can do for
yourself. It is this type of information
that we will attempt to provide to you. This will vary from person to person but
will probably include
prescribed exercises, the use of ice, better body mechanics and dietary tips
as well as ways to reduce stress
in your life, and any limiting or increasing of activities. It is expected that
these guide lines will be followed and
the results be reported upon on return visits.
Dr. Rex’s hope is that his help to patients would be in the line of preventive
maintenance and education so
that the patient’s life can be fuller and healthier with fewer day to day
medical and musculoskeletal problems.
Dr. Rex wants all of his patients to have the following information:
Our office does not accept any medical plans or medical insurance.
We do not accept patients with
Labor and Industry cases, accident victims involved in litigation, Welfare, or
Medicare or Medicare age eligible.
We only accept patients between the age of 16 and 64. We do not accept
attorneys or members of their
immediate families. We do not accept checks from insurance companies.
Patients will pay as they receive services at each visit. You may pay cash or
with personal check we do not
accept credit cards. A statement showing payment will be sent or given to the
patient. Our office does not
communicate with insurance companies. We do not write letters, make reports or
fill out forms.
This is not negotiable. We make no exceptions.
We suggest that you do not anticipate being reimbursed by your insurance
company, if you do get any
reimbursement you will have a pleasant surprise. By making an appointment you
are accepting the rules
and conditions of our office.
Fees First Visit $400.00 2
hours with the doctor
Follow up
visit $200.00 1 hour (minimum)
Patients who miss their first appointment without notice of cancellation or if
you miss two follow up
appointments without giving notice of cancellation will not be given future
appointments.
You will be given a patient history form. Bring this completed form with you to
your first visit.
If you have current x-rays or laboratory results bring them with you. Your
appointment time represents
the time you should be in the exam room ready for treatment. Please come a few
minutes early to help
